What is Enhanced Care Management?

Enhanced Care Management (ECM) is a high-touch Medi-Cal benefit for eligible members with complex health and health-related social needs. Anchor provides a single point of coordination so the member, providers, health plan, and community partners can work from one person-centered care plan.

Who may be a good fit?

ECM may be appropriate for people experiencing homelessness, frequent hospital or emergency-department use, complex physical or behavioral health needs, or difficult transitions between systems of care. The member’s managed care plan makes the final eligibility and authorization decision.

What does an ECM Lead Care Manager do?

The Lead Care Manager serves as the member’s primary coordinator, helping organize care, follow up on referrals, reduce duplicated effort, and connect clinical care with housing and social supports.

Who can refer someone for ECM?

Health plans, hospitals, clinics, behavioral health teams, community organizations, and other authorized referral partners may send a referral. Members and families can also contact Anchor so our intake team can identify the correct route.

Does ECM include housing?

ECM is care management rather than a housing benefit. Eligible members may also qualify for separate Community Supports such as Housing Transition Navigation, Housing Deposits, or Housing Tenancy and Sustaining Services.

How quickly will Anchor review a referral?

Anchor typically screens complete referrals in under two business hours. Authorization timing depends on the member’s health plan and the information provided.
