SHORT-TERM RECOVERY

Recuperative Care & Medical Respite

Short-term residential care for people with unstable housing who no longer need hospitalization but still need a safe place to heal. Services include housing, meals, monitoring, transportation coordination, and recovery support.

Short-Term Post-Hospitalization Housing

Housing for eligible members without a residence who have significant medical, behavioral health, or substance-use recovery needs after leaving an inpatient or residential setting. The benefit can support recovery for up to six months.

Mentoring and practical coaching for eligible members who are experiencing, at risk of, or formerly experienced homelessness. Support can include transportation skills, cooking, cleaning, finances, communication, and community living.

Sobering Center Services

A short-term, safe, supportive setting for eligible members who are publicly intoxicated to become sober, receive triage and basic support, and connect with appropriate health and substance-use services. Access is coordinated according to the member’s county, health plan, clinical appropriateness, and current program capacity.

Sober Living & Long-Term Co-Living

Furnished, all-inclusive housing for adults committed to a drug- and alcohol-free environment or seeking a stable long-term room. This direct housing path is separate from a Medi-Cal referral.

How referrals work

1. Send the referral

Use the secure intake options on the Referrals page. Include the discharge summary and current medication list for recuperative care referrals.

2. We review fit and availability

Our intake team screens referrals in under two business hours and confirms clinical readiness, authorization, program fit, and bed availability.

3. The member moves in

We coordinate transport when applicable. The member arrives to a furnished home and program support begins.

What an Anchor home can include

  • A furnished private or shared room
  • Bed, linens, dresser, utilities, laundry, and Wi-Fi
  • Nutritious meals; medically tailored meals where indicated
  • A clean, drug- and alcohol-free environment
  • House rules, professional oversight, and community support
  • Care coordination, medication reminders, and appointment support when included in the program

Housing is the foundation. Care coordination and tenancy support help members build stability around it.

Enhanced Care Management

Enhanced Care Management (ECM) is a Medi-Cal benefit for eligible members with complex needs. Anchor’s care team connects the medical, behavioral health, housing, benefits, and social-service pieces of a member’s plan so they do not have to navigate each system alone.

ECM may include comprehensive assessment and care planning, coordination with providers and health plans, help attending appointments, connections to community resources, and support during transitions between hospitals, programs, and housing.

Housing Transition & Tenancy Support

Housing support helps eligible members move from homelessness or unstable housing toward a lasting home. Depending on authorization, Anchor can help identify housing, complete applications, gather documents, coordinate deposits, prepare for move-in, communicate with landlords, and address problems that could put a tenancy at risk.

For members leaving recuperative care, this work can begin before discharge so the next step is planned rather than improvised.

Do I make a referral or apply for housing?

Hospitals, health plans, county teams, and other partners should use the referral intake hub. Individuals looking for sober living or a long-term room can apply for housing directly.

What areas do you serve?

Anchor operates 18 homes across 20 California counties. Availability changes by program and location. We also maintain a dedicated resource for recuperative care in Los Angeles County.

How quickly are referrals reviewed?

Which health plans work with Anchor?

Is Enhanced Care Management the same as a Community Support?

No. ECM is a separate Medi-Cal care-management benefit. Community Supports address health-related needs such as recovery housing, navigation, deposits, tenancy support, and independent-living skills. Eligible members may receive both.

Are all services available in all 20 counties?

No. The 20-county figure describes Anchor’s contracted network. Plan participation, authorization, operational capacity, and bed availability vary. Intake confirms the available service pathway for each member.

Is Anchor a medical facility?

No. Anchor homes are not licensed medical facilities and do not provide clinical treatment on site. Home health providers may visit during normal hours, and Anchor staff coordinate non-clinical recovery support.

Who can be referred for recuperative care?

Adults should be medically stable, able to perform activities of daily living independently or with minimal assistance, and willing to follow a drug- and alcohol-free environment. The intake team confirms individual fit.
