Questions about service areas

Does a listed county guarantee placement?

No. A listed county confirms a contracted relationship. Eligibility, authorization, requested service, provider capacity, and available housing or program slots still apply.

Why are some services available in fewer counties?

Managed care plans elect and contract for Community Supports individually. Day Habilitation and Sobering Center services currently have narrower contracted footprints than Anchor’s housing-related services.

Can I refer from outside these counties?

Contact intake if the member’s location, health plan, or discharge destination is unclear. We will identify whether an Anchor contract or another appropriate pathway may apply.

What information should a referral include?

Include member demographics, Medi-Cal plan, county, requested service, relevant clinical or housing documentation, current location, and a reliable contact for follow-up.
