What is Day Habilitation?

Day Habilitation is a CalAIM Community Support for eligible members who are experiencing homelessness, at risk of homelessness, or formerly experienced homelessness. Anchor provides individualized, hands-on coaching in the member’s home or another appropriate non-facility setting.

Who may be a good fit?

Day Habilitation may fit a member who needs structured coaching to manage daily routines, communicate effectively, use community resources, and build the skills required for greater housing stability and independence. The managed care plan makes the final eligibility and authorization decision.

HOW TO START

A clear referral path

Send the referral through Anchor’s secure intake hub. Our team reviews the member’s plan, county, requested service, documentation, and urgency, then coordinates the appropriate authorization path.

Complete referrals are typically screened in under two business hours.

Where are Day Habilitation services delivered?

Services may be provided in the member’s home or another appropriate community-based, non-facility setting, based on the authorized plan and the skills being practiced.

Goals may involve self-help, social, adaptive, household, transportation, communication, scheduling, or community-navigation skills that support successful independent living.

Who is eligible?

The service is designed for eligible members who are experiencing homelessness, at risk of homelessness, or formerly experienced homelessness. The managed care plan determines final eligibility and authorization.

Is Day Habilitation available in every Anchor county?

No. Anchor is contracted for this service in Contra Costa, Napa, Placer, Solano, Sonoma, and Yolo counties. Capacity and authorization still apply.
