What is Short-Term Post-Hospitalization Housing?

Short-Term Post-Hospitalization Housing (STPH) is a CalAIM Community Support for eligible members with significant medical, behavioral health, or substance-use recovery needs who do not have a residence after discharge. When authorized, the service can provide housing for up to six months while the member continues recovery and works toward a longer-term plan.

Who may be a good fit?

STPH may fit members leaving an inpatient hospital, residential substance use disorder treatment or recovery facility, residential mental health treatment facility, correctional facility, nursing facility, or recuperative care. The member must meet plan-specific eligibility and authorization requirements.

How long can Short-Term Post-Hospitalization Housing last?

DHCS describes STPH as short-term housing for up to six months. The actual authorization period and continued eligibility are determined by the member’s managed care plan.

What discharge settings can qualify?

Qualifying settings can include inpatient clinical care, residential behavioral health or substance-use treatment, correctional facilities, nursing facilities, and recuperative care, subject to current DHCS and health-plan requirements.

Is STPH the same as recuperative care?

No. Recuperative care is designed for people who still need a safe place to heal from an acute illness or injury after hospitalization. STPH supports a broader continued recovery and housing transition after qualifying settings.

What should a referral include?

Send member demographics, health-plan information, the discharge setting and date, relevant clinical documentation, current housing status, and a reliable contact for care coordination.
