What is Short-Term Post-Hospitalization Housing?
Short-Term Post-Hospitalization Housing (STPH) is a CalAIM Community Support for eligible members with significant medical, behavioral health, or substance-use recovery needs who do not have a residence after discharge. When authorized, the service can provide housing for up to six months while the member continues recovery and works toward a longer-term plan.

Send the referral through Anchor’s secure intake hub. Our team reviews the member’s plan, county, requested service, documentation, and urgency, then coordinates the appropriate authorization path.
