What is Housing Transition Navigation?

Housing Transition Navigation Services are a CalAIM Community Support for eligible members experiencing or at risk of homelessness. Anchor helps turn a housing goal into an actionable plan—from document readiness and applications to landlord engagement and move-in coordination.

Who may be a good fit?

Navigation may be appropriate when a member needs more than a housing list: they need hands-on help completing steps, resolving barriers, coordinating benefits, and staying connected through the housing search.

HOW TO START

A clear referral path

Send the referral through Anchor’s secure intake hub. Our team reviews the member’s plan, county, requested service, documentation, and urgency, then coordinates the appropriate authorization path.

2. Anchor screens the request
Complete referrals are typically screened in under two business hours.

3. We coordinate the next step
Eligibility and authorization are confirmed with the managed care plan before services begin.

Questions about Housing Transition Navigation

What is included in housing navigation?

Services can include assessment, planning, document preparation, housing searches, applications, landlord communication, benefit coordination, and support through lease-up or move-in.

Can Anchor help someone who has a voucher?

Yes. When authorized, navigation can help with housing search, applications, landlord engagement, and coordination of the steps needed to use an available voucher.

Does the member have to be homeless?

Eligibility may include members currently experiencing homelessness or at risk of homelessness. The managed care plan applies the current DHCS criteria and makes the authorization decision.

Can navigation be combined with Housing Deposits?

Yes, when the member is separately eligible and authorized for both services. Navigation helps secure housing; Housing Deposits may cover specific eligible one-time move-in costs.
